HonorVet Technologies is seeking a travel Home Health Physical Therapist for a travel job in Portland, Oregon.
Job Description & Requirements Title: Physical Therapist – Home Health
Location: Portland, OR (East Portland and surrounding areas)
Duration: 13 weeks
Shift: Day shift, 5x8-hour shifts (8:00 AM – 4:30 PM)
Provide one-on-one physical therapy services to patients in a home health setting.
\tPerform comprehensive patient evaluations, reassessments, and discharge planning in patients' homes.
\tDevelop, implement, and update individualized plans of care and home exercise programs.
\tComplete required documentation including evaluations, OASIS assessments, reassessments, short- and long-term goals, and discharge summaries.
\tTreat adult and geriatric patients with a wide range of diagnoses including dementia, Parkinson's, ALS, MS, post-stroke conditions, and post-operative orthopedic cases.
\tManage a daily productivity expectation of approximately 5.5–6 points per day, with up to 6 patient visits per day.
\tEducate patients and families on safety, injury prevention, disease processes, and functional mobility.
\tAssess and address environmental barriers and safety risks within the home.
\tUtilize assistive devices and adaptive equipment to support patient mobility and independence.
\tTravel independently between patient homes; facility groups visits to minimize drive time, with occasional travel up to a 60-mile radius in a day.
\tCollaborate with the interdisciplinary care team as needed.
\tRequired Qualifications:
Minimum of 1 year of skilled home health physical therapy experience.
\tPrior home health experience required.
\tOASIS experience required (must be disclosed if not).
\tValid Physical Therapist license (pending license accepted).
\tBLS certification.
\tAbility to work independently in a home health environment.
\tReliable personal transportation.
\tRTO must be communicated per facility guidelines.
\tAbility to meet documentation and productivity standards.
\tAdults and geriatrics patient care experience.
\tPreferred Qualifications:
TDN certification.
\tExperience across multiple care settings including acute care, outpatient, LTAC, inpatient rehab, and SNF.
\tExperience with Epic (preferred).
