Title: Physical Therapist – Home Health
Location: Portland, OR (East Portland and surrounding areas)
Duration: 13 weeks
Shift: Day shift, 5x8-hour shifts (8:00 AM – 4:30 PM)

Job Responsibilities:
    \t
  • \t

    Provide one-on-one physical therapy services to patients in a home health setting.

  • \t

    Perform comprehensive patient evaluations, reassessments, and discharge planning in patients' homes.

    \t
  • \t
  • \t

    Develop, implement, and update individualized plans of care and home exercise programs.

    \t
  • \t
  • \t

    Complete required documentation including evaluations, OASIS assessments, reassessments, short- and long-term goals, and discharge summaries.

    Treat adult and geriatric patients with a wide range of diagnoses including dementia, Parkinson's, ALS, MS, post-stroke conditions, and post-operative orthopedic cases.

    Manage a daily productivity expectation of approximately 5.5–6 points per day, with up to 6 patient visits per day.

    Educate patients and families on safety, injury prevention, disease processes, and functional mobility.

    Assess and address environmental barriers and safety risks within the home.

    \t
  • \t
  • \t

    Utilize assistive devices and adaptive equipment to support patient mobility and independence.

  • \t
  • \t

    Travel independently between patient homes; facility groups visits to minimize drive time, with occasional travel up to a 60-mile radius in a day.

    Collaborate with the interdisciplinary care team as needed.

Required Qualifications:

    Minimum of 1 year of skilled home health physical therapy experience.

    \t
  • \t
  • \t

    Prior home health experience required.

    \t
  • \t
  • \t

    OASIS experience required (must be disclosed if not).

    \t
  • \t
  • \t

    Valid Physical Therapist license (pending license accepted).

    \t
  • \t
  • \t

    BLS certification.

    \t
  • \t
  • \t

    Ability to work independently in a home health environment.

    \t
  • \t
  • \t

    Reliable personal transportation.

    \t
  • \t
  • \t

    RTO must be communicated per facility guidelines.

  • \t

    Ability to meet documentation and productivity standards.

  • \t

    Adults and geriatrics patient care experience.

Preferred Qualifications:

    TDN certification.

    Experience across multiple care settings including acute care, outpatient, LTAC, inpatient rehab, and SNF.
